  • Requires on call commitment for 7 days every third week
    • Job Requirements
    • BLS - Required for clinical positions
    • License/Certificate Verification - Primary Source Verification of professional license and/or certification
    • MMR - Positive MMR titers ONLY from within the last 15 years
    • Fit Test - Completed within 1 year of start
    • Hepatitis B - Proof of vaccination or signed declination
    • TDAP - Proof of vaccination within past 10 years
    • UHS I-9 Attestation - Agency to provide completed I-9
    • UHS Education Verification - Highest level of education verified
    • UHS Employment Verification - Must provide verification of employment within 5 years
    • UHS Skills Checklist - Clinical skill specific required to be complete within 1 year
    • UHS Competency Exam - Must score at least 80%
    • UHS Core Competencies - 80% passing score required
    • Physical/Health Statement - Pre-Employment exam statement required
    • Influenza Vaccine - Proof of vaccine from Oct-March or exemptions must be submitted for approval
    • 2 Step PPD OR IGRA (Quantiferon/Tspot) - 2-Step TB test with negative reads within last 6 months
