Job Description:
A company is looking for a Telephonic Care Navigator to provide
support and outreach to members remotely in Connecticut.
Key Responsibilities:
Conduct telephonic outreach to members and assist with care gaps
and appointment scheduling
Link members to community resources and ensure access to primary
care providers
Meet performance metrics while maintaining confidentiality and
providing excellent customer service
Required Qualifications:
High School Diploma / GED
At least 1 year of call center or telephonic customer service
experience
1+ years of experience in healthcare, insurance, or community
outreach
Proficient in Microsoft Word, Excel, and Outlook
Ability to work full-time during standard business hours with
potential for overtime
